New Guidelines and Clinical Tactics for Optimal Management of Dyslipidemia in Patients with or at Risk for Cvd*

چکیده
Cardiovascular disease (CVD) is the leading cause of death in the United States as well as in other highly developed countries. It is also the leading cause of death in people with diabetes, a disease becoming rapidly more prevalent. This article reviews a series of clinical trials showing the benefits of lipid-lowering agents in reducing CVD risk. It also provides a review of the recent treatment guidelines, including the Report of the National Cholesterol Education Program Expert Panel on Detection, Evaluation, and Treatment of High Blood Pressure in Adults and focuses on the primary prevention in patients with multiple risk factors. Additionally, appropriate management of special populations is analyzed, including patients with the metabolic syndrome, elderly patients, those with low levels of high-density lipoprotein cholesterol, and those with coronary artery syndrome. Also analyzed is the risk assessment value of the high-sensitivity C-reactive protein assay in clinical practice. Overall, this article provides evidence that lipid treatment should be part of the overall approach to managing CVD risk factors. (Adv Stud Med. 2004;4(5A):S372-S381) N umerous preventive therapies have been shown to reduce cardiovascular disease (CVD) risk, and any approach to CVD prevention in high-risk patients should address all risk factors. The use of lipidlowering agents, statins in particular, to lower CVD risk substantively is well documented, and the number of patients who can benefit from therapy continues to expand as more clinical data accrues. This approach includes appropriate behaviors and therapies, such as a low-fat diet (with fats limited to omega-3 fatty acids), aerobic exercise, smoking cessation, blood pressure (BP) control, aspirin, and beta blockers. Including lipid-lowering agents in this composite approach has also proved to be of substantial help in preventing CVD progression and related events. A number of clinical trials have shown that the use of lipid-lowering agents should be a part of a multifaceted approach to CVD prevention.

Evaluation of Guidelines and Risk Factors for Venous Thrombosis and Pulmonary Embolism in Hospitalized Children: A Cross-Sectional Study
Background: Thromboembolism (TE) in pediatric population is rare but may be a fatal situation. There is a lack of clinical guidelines to help decision making for the use of prophylactic measures in pediatrics. This study was designed to evaluate current guidelines and risk factors for the prevention of venous thrombosis and pulmonary embolism (DVT/PE) in children. Materials and Methods: Th...

Background: Type 2 diabetic patients have higher risk for death from coronary heart diseases than non-diabetic patients. Studies have revealed showed intensive treatment of hyperglycemia only have a small effect on CVD risk. Other risk factors such as Lipid abnormality play a leading role in the increased CVD risk associated with diabetes. The aim of this study was to assess the quality of lipi...
متن کاملApproach to Lipid Screening as a Risk Marker for Cardiovascular Disease in Pediatric Patients with Diabetes
Cardiovascular disease (CVD) is a well-known complication of diabetes mellitus (DM), and patients with DM are at an increased risk for early onset of CVD. Hyperglycemia is believed to be the primary mediator in premature development of atherosclerosis in patients with DM, but there are also derangements in cholesterol levels and inflammatory markers beyond the explanation of hyperglycemia. Alth...
متن کاملPrevalence of Aspirin Use among Type 2 Diabetic Patients in Iran
Abstract Objective: Patients with type 2 diabetes mellitus (DM) have a markedly increased risk of cardiovascular morbidity and mortality. There are some recommendations for prescription of aspirin in these patients. Our purpose was to determine aspirin usage rate in diabetic patients and to compare it in different ages and based upon cardiovascular disease (CVD) risk factors. Materials and M...
متن کاملManagement of lipid disorders in patients living with HIV.
Since the discovery and development of antiretroviral therapy (ART), HIV has become a chronic disease with patients living longer lives and to ages where co-morbidities, such as cardiovascular disease (CVD) are prevalent. Diagnosis and management of risk factors for CVD, in particular dyslipidemia, have become an important part of the overall care for patients living with HIV infection. Existin...
